上海市金山区居家严重精神障碍患者用药依从性影响因素及药

摘 要 目的 分析居家严重精神障碍患者用药依从性现状、影响因素及药物不良反应发生情况。方法 整群抽取2022年1月上
海市金山区纳入上海市精神卫生信息管理系统的居家严重精神障碍患者为研究对象，结合信息系统登记的相关基础数据并通过
精神卫生条线医生面对面或电话开展问卷调查，了解患者的基本情况、用药依从性、精神症状、社会功能缺陷状况、药品使用信息
和药物不良反应发生情况，并进行用药依从性的单因素和多因素Logistic回归分析。结果 本次调查发放问卷2 684份，回收有效
问卷 2 606 份，有效回收率为 97.10%。居家严重精神障碍患者整体用药依从性为 68.11%。多因素 Logistic 回归分析结果显示，
51～60岁、未婚、离婚或丧偶、社会功能部分缺陷、社会功能无缺陷、使用第3代抗精神病药品是患者用药依从性的促进因素（P＜
0.05）；费用类型为医保、不参与社会工作、住院总次数1～4次、住院总次数≥5次、精神症状一般、使用第2代抗精神病药品是患者
用药依从性的阻碍因素（P＜0.05）。居家严重精神障碍患者药物不良反应发生率为10.59%，主要以第2代抗精神病药品为主，无
新的、严重的不良反应上报。结论 居家严重精神障碍患者的用药依从性受多种因素影响，亟需药师的参与，以促进患者用药的规
范、合理。

ABSTRACT OBJECTIVE To analyze the status quo，influential factors of medication compliance and adverse drug reactions of
home-based patients with severe mental disorders. METHODS Patients with severe mental disorders at home who were enrolled in
the Shanghai mental health information management system in Jinshan district of Shanghai in Jan. 2022 were selected as the
research objects. In combination with the relevant basic data registered in the information system and through face-to-face or
telephone surveys by mental health doctors，basic information，medication compliance，mental symptoms，social function defects，
drug use information and adverse drug reactions of patients were understood. Univariate and multivariate Logistic regression analysis
of medication compliance were performed. RESULTS Totally 2 684 questionnaires were distributed and 2 606 valid questionnaires
were recovered，with an effective recovery rate of 97.10%. The overall medication compliance of patients with severe mental
disorders at home was 68.11%. The results of multivariate Logistic regression analysis showed that 51-60 years old，unmarried，
